With the blessings of   Guruhari  Hariprasad  Swami  Indian Health Camp of New Jersey is proudly organizing Health Screening Camp for the uninsured people over the age 50 and people with acute problem under age 50.

The following screenings will be provided to the first 125 participants on a first come first served basis.

Deadline for the submission of this form is September 14, 2008.
You must pre-register, as walk-in will not be allowed.  Please fill-out one application per person.

Note: 12 hours fasting is required for blood test.   Continue all medications the day before and the day of the blood test.   Bring all your medications with you on the day of physical examination.  Breakfast will be provided after the blood test is completed. 

• Planned Health Screening (Actual test/screening will be determined by the physician on case-by-case base):
• Complete Blood work
• Electro Cardiogram (EKG)
• Physical Examination
• Heart Disease Screening and
• Vision Testing.

*** Patients may need to attend another location for this test.  We will notify on the day of the camp.
